Cancellation Fee: As a courtesy to our patients and staff, we kindly request to give us 48 hours advanced notice to cancel or reschedule appointments. Patients who do not show up to their appointments or cancel within 48 hours of their appointment time may be subject to a fee of $50 starting January 1, 2023.
We currently take the following PPO plans: Delta Dental, MetLife, Cigna, Aetna, Anthem BlueCross, Blue Shield of CA, Guardian, United Healthcare, SunLife/Dentaquest, GEHA, and Premier Access. For any PPO Plan not listed, please inquire with our office or your insurance provider.
We are not in-network with HMO plans or Medi-care/MedicAID plans.
When making your initial appointment, we will need the Name of your Insurance Company, Subscriber ID Number, and Date of Birth to verify your insurance ahead of time.
We accept the following self-pay options: cash, checks, and credit card. We also offer interest free financing options through CareCredit
Yes, dental expenses are qualified FSA/HSA expenses. Please refer to this link for a list of qualified expenses.

Yes, there is an ADA entrance at 417 Grand Avenue that is connected to 415 Grand Avenue. ADA parking is located on Grand Avenue.
Is there ADA Accessibility?
There is ample street parking that is metered from 8am-8pm Monday-Saturday. There is an additional parking structure located on the next block with it's entrance on Miller Ave.

I have a dental emergency, how can I schedule an appointment?
Yes, Dr. Liao can see a patient as early as 6 months to get them acclimated to the Dental Office.
